Hammer and forge missing chapter??
Did i miss a chapter somewhere? Our hero was supposed to meet Thraxis some time in the past after losing his status as knight. Did I miss when this occurred or did the story just end before it occurred?

They met in the past during time watch arc.

There's more than one hole in the story. For instance, Elias Harkonnen was supposed to go to Center two times, once when he had both eyes, and later after he'd lost one somehow. Well, he lost one eye in the final fight with Caleb Vulcan, but never got around to that second trip to Center.
This is just one example, but there are others.
I can think of two explanations: 1) Time travel changes things in the past, thus altering the future as the time traveler knew and predicted it, or 2) the author lost track of past story threads or just got tired of the whole thing and brought the story to a close. Unfortunately there aren't any definite clues in the HotF to tell us one way or the other. The obvious problems with the story timeline, along with the fact that there would need to be a MAJOR edit so that the same characters aren't being re-introduced every 5-10 pages, are the reasons why I am against the whole thing being compiled into a stand alone book for PB to publish. I just think their time and energies would be better spent elsewhere.
But that is just my opinion.

Thraxus commented that he met Vulcan 3 times. Once during the battle of the watch arc, once when he'd fallen, then in the current time when he nearly killed him.

Since Caleb didn't lose his knight status until the end of the book I would assume if he still meets Thraxis after that it would be after the end of the book.
