I'm not seeing it or remembering ever seeing it, neither the agony thing or the initials thing.
Sounds like house rules your GM (or you if you are GM) misremembered and thought was cool. Happens to the best GM's. I know I've had moments where I realized years later a rule I took for granted as canon had been a house rule the entire time.
But by default, if a demon lord wants to Agony somebody, they have to cast the spell like normal and the target gets a saving throw as normal.
does anyone remember which OCC of demons can be Agonied?
